Adult ADHD Self-Report Scale (ASRS-v1.1), full 18 questions

The complete WHO Adult ADHD Self-Report Scale v1.1: Part A, the six-question screener, and Part B, twelve further questions on inattention and hyperactivity. Reproduced verbatim. A screening result is not a diagnosis; only a qualified clinician can diagnose ADHD.

Before you start. This is a screening questionnaire, not a diagnosis. Your eighteen answers are stored in a separate table, under a consent you can withdraw. The result is shown to you only: it never appears on a share card, never reaches your agent, and is never used to recommend anything. You can delete it at any time. Source: ASRS-v1.1 Symptom Checklist, World Health Organization, developed with Harvard Medical School and New York University Medical School (Kessler et al., 2005).

Where to turn: Malaysia: your general practitioner or a registered clinical psychologist; the Malaysian Mental Health Association (mmha.org.my). Elsewhere: your primary care doctor. In an emergency, contact local emergency services.
